Any communication from the VPK program will be posted on this site. You may check this site as often as you like to see what communications have been sent to VPK providers.
Since neither email, regular mail, nor fax has successfully reached all VPK providers, you now have a place where you can check to see what is going on with VPK.

Completed AWI-VPK-10 for summer if doing a summer program and one for school year with original signature Authorized Signature Form notarized with original signature. See direction sheet previously sent to you. Advance vs. Actual Prepayment form with original signature. Please bring these forms in to your Assigned VPK Registrar IMMEDIATELY. If these forms are not received by the deadline, there is NO GUARANTEE that your school will be on the first provider list.

If you have not submitted your request, you can come in and we will pack them quickly for you.
