Family Central is happy to announce that we now have the service agreement information packet available online. All providers interested in participating in the service agreement contract, must complete the service agreement information packet.
If you did not have a service agreement contract last fiscal year, 2008/2009, you must complete a full packet. Providers, who had a service agreement contract last fiscal year, must complete the service agreement amendment information packet.
All providers who had an existing contract last fiscal year will be receiving a service agreement amendment information packet in the mail. Please complete the packet as usual and submit it with all required documents, or you may submit the information packet by e-mail to Family Central or fax to (954)724-3936.
Providers may call for an appointment to execute the contract prior to submitting the packet. Any provider submitting their packet by mail or electronically will be contacted by the service agreement staff to arrange an appointment to execute the contract.

There are no charges/fees associated with a provider listing in the Child Care Resource & Referral Database or for referrals to your program. If you are asked to provide a payment for a referral or listing in the Child Care Resource & Referral database, please contact the Agency for Workforce Innovation’s Office of Early Learning or call at 1-866-357-3239. The information reported about a provider’s program is objective program information that is based on standards that are attainable for providers.
