Thank you to all providers who attended the school readiness (SR) and voluntary prekindergarten (VPK) training session on Friday, June 3rd, 2011.   We appreciate your participation and interest in serving children and their families in Broward County.

Service Agreement for School Readiness Providers

Thank you for your interest in participating as a School Readiness Provider!  Child care providers in South Florida interested in serving Broward County School Readiness funded children and their families must first complete an Application for a Service Agreement. 

PLEASE NOTE:  Applications are now due.  To facilitate a smooth transition for all payments effective July 1, 2011, submit your application and supporting paperwork today. 

Please complete the application, include copies of all requested documents and deliver to the Service Agreement Department at Family Central, Inc.    For your convenience, you may deliver it via one of the following ways;

Each application will be reviewed for current documents and appropriate signatures.  Once approved, applicants will receive their Service Agreements electronically to sign and return to FCI.  Applicants are not considered School Readiness Providers until two original-signature Service Agreements are signed and returned to FCI.

Any required documentation that expires during the fiscal year will need to be immediately updated or the Service Agreement will become suspended.  Please remember to keep all your documents current with the Service Agreement Department. 

The Service Agreement is our contract with you to deliver school readiness services to children and their families and allows us to reimburse you accordingly.

If you are asked to provide a payment for a referral or listing in the Child Care Resource & Referral database, please contact the Agency for Workforce Innovation’s Office of Early Learning or call at 1-866-357-3239. The information reported about a provider’s program is objective program information that is based on standards that are attainable for providers.
